The Limited Access Death Master File Certification Application Instructions, Limited Access Death Master File Subscriber Certification Form, Limited Access Death Master File Subscriber Agreement, and Limited Access Death Master File Licensee Agreement for Use and Resale have been temporarily removed. Expiration of Paperwork Reduction Act (PRA) Clearance of the DMF Certification Form

Please note that the Emergency PRA Clearance for the DMF Certification form expires on September 30, 2014. At the close of business on September 30, 2014 the form and all related instructions will be removed from the NTIS website, and NTIS may not accept certifications from that time until the NTIS receives Office of Management and Budget (OMB) clearance for DMF Certification form under the regular PRA process. OMB cannot approve the DMF Certification under the regular PRA process until that process is completed. The first step in the process, a 60-day comment period announced in the Federal Register on August 22, 2014, will be complete on October 21, 2014. The remainder of the process is expected to take approximately 45 additional business days. NTIS will update this website with further information as the process progresses. Persons certified prior to September 30, 2014 may continue to have access to the Limited Death Master File during the interim period.

Request for Comments on the DMF Subscriber Certification Form

NTIS published a notice in the Federal Register on August 22, 2014 (79 FR 49755), requesting comments on the DMF Subscriber Certification form. Interim Final Rule Available

The Interim Final Rule, "Temporary Certification Program for Access to the Death Master File," was published in the Federal Register (79 FR 16668) and took effect on March 26. It may be reviewed at http://www.gpo.gov/fdsys/pkg/FR-2014-03-26/pdf/2014-06701.pdf.

Limited Access Death Master File Certification Process Now Open

To apply for certification to receive the Limited Access Death Master File before the PRA clearance for the DMF Certification form expires on September 30, 2014 go to Limited Access Death Master File Certification Application Instructions. To obtain access to the Limited Access Death Master File, you must submit your certification form and agreement to dmfcert@ntis.gov by September 30, 2014.
